• During the enrollment call, the Social Security Number and date of birth are required for all NEW dependents (spouse and/or children), and you will be asked to designate beneficiaries for your employer-provided life insurance policy. Domestic Partners are ineligible.
• You must email proof of eligibility for each NEW dependent, such as a birth certificate or marriage license, to Jenien Desilvis, HR Benefits Specialist at [email protected]. 
• All personal information changes must be submitted to HR via email to Jenien Desilvis at [email protected]

If you are experiencing a Qualifying Event, you must contact a benefit counselor through the Call Center to START the QLE process within 30 days of the event. The benefit counselor will provide you with information on what qualifies for a QLE and process the changes in the system for you. Once changes are made in the system, you will have to contact Jenien Desilvis, HR Benefits Specialist at [email protected], to finalize the QLE process and provide proof of documentation. HR will provide you with the FINAL approval via email after receiving the pending documentation. When connecting with a Benefits Counselor, mention that your call is due to a QLE (loss of healthcare coverage/birth/marriage).

Please note: You can modify your benefit elections only during Active Open Enrollment through the Call Center or if you experience a Life Event. However, you may update beneficiaries and download your benefit confirmation statement.
