If you are experiencing a Qualifying Event, you must contact HR within 30 days of the event to process it and provide proof of documentation.QLE changes are processed through Human Resources. If you are connecting with a Benefits Counselor with questions about a Qualifying Life Event, mention that your call is due to a QLE (loss of healthcare coverage/birth/marriage).

After your enrollment, download the applicable forms and email the Benefits Specialist, Jocelyn Torres at [email protected] within 30 days of enrollment!
⏭️During the call, the Social Security Number and date of birth are required for all NEW dependents (spouse and/or children). Domestic Partners are ineligible.
⏭️You must email the Benefits Specialist proof of eligibility for each NEW dependent, such as a birth certificate or marriage license within 30 days of enrollment!!
⏭️All personal information changes must be submitted to the Benefits Specialist by June 5th!
⏭️Verify –you will be asked to designate beneficiaries for your employer-provided life insurance policy.
If needed, when filling out your Evidence of Insurability (EOI) within 30 days of enrollment, use the following information:
